I don’t know if your problem has been resolved; however, I had the same issue when updating the firmware to 4.16. I found that when you upgrade to 4.16, it overwrites the default radio drivers and corrupts the driver names. The modem cannot find the radio files and loads a generic driver that does not work for all tasks. When I tried to upgrade the radio firmware, it failed.
What did work was to add the correct radio driver to the system. If you goto Admin/Radio modem files, you can’t delete the bad files; however, if you try to add the file, it will overwrite the bad file and put in the correct name. After that, it works. Before doing this, I constantly got the memory read error.
