Missing SIM Information

I currently have an issue where an RV50 modem has been deployed, is successfully working, but the AirVantage cannot see the firmware remotely (thus telling me to upgrade). I scroll down and find both the SIM card and Phone Number fields blank.

1, How did I manage to get this activated without that information?
2, Is there a way to correct the information now so that the modem can sync to airvantage?
3, What is the correct way to do this in the future so I don’t make this mistake again?

The SIM card is from Verizon Wireless. I didn’t know to order any specific kind (although that may be part of the issue.
